Inequality in the Medical Care Utilization and Medical Expenditure for the Patients with Chronical Illness

Abstract
We measured the degree of inequalilty of the medical care utilization and medical expenditure for the patients with chronical illness such as high blood pressure, diabetes and arthritis. We also analyzed the factors which determine the inequality. In measuring the degree of inequality, we use the concentration index, HIwv index and the concentration index decomposition. Regression analysis found that the concentration index for the inequality in the medical utilization showed negative sign, which implied pro-poor, meanwhile HIwv index which reflected medical needs was positive, which implied pro-rich. The factors which determined the medical utilization were income, economic activity, age, education and private health insurance, etc. Concentration index and HIwv index for the inequality in the medical expenditure showed positive sign which implied pro-rich. The determining factors which affected medical expenditure were health insurance system, age, economic activity and duration of illness, etc. To solve the inequality in the medical utilization and medical expenditure, the benefits of the national health insurance system should be expanded(raised) to reduce the copayment of the patients
